I hereby agree that the Czech Cooperative Group for Head and Neck Cancer, z.s. (hereinafter referred to as the "Company"), with its registered office in Prague 5, V Úvalu 84, 150 06, processed and recorded my personal data provided in connection with the application for membership and the activities of the Association, pursuant to Act No. 101/2000 Coll., on the Protection of Personal Data (as amended).
I further declare that I am aware of my rights under Sections 12 and 21 of Act No. 101/2000 Coll., In particular the right to contact the administrator with a request for information about the processing of the provided personal data, with a request for explanation in the case of doubts about the processing of provided personal data in accordance with the law and with the requirement to remove such a situation, for example, by repairing, supplementing or disposing of personal data.
I give this consent on a voluntary basis, for the time necessary to complete my application and for the duration of my membership, and I can withdraw it at any time.
